Section 1: Brood Bitch
- Elbow clearances by age 2
- Breeding not more than two times in the course of four consecutive menstrual cycles (i.e., not more than twice in about two years)
- Thyroid clearance annually (Within a year of breeding) or other period of time
- Eye clearance annually with certification of free of entropion/ectropion or other period of time
- Cardiac clearance
- vWD clearance
- Breeding not before the age of 24 months
- Breeding only champion (relevant to clubs of certain countries, e.g., AKC/CKC )
Section 2: Stud Information
- Elbow clearances by age 2

Section 3: Breeder Information
- Evidence of commitment to puppies for life
- kennel to 3 - 5 - 7 (??) litters per year (breeder fill in average here____)
- Serves as mentor in our breeder mentoring program
- Positive veterinary statement of exemplary breeding practices and kennel management
- Temperament testing of puppies/selection
- Temperament testing of adults before breeding (ATT)
- Interest in formal obedience (&competition), drafting, herding, tracking, agility, S&R, therapy (breeder to list ALL applicable)
